One-Step Questionnaire/Summons
Please complete both Part B- Qualification Questionnaire section and Part C- Juror Personal History Questionnaire sections. If sending via mail, please detach and retain
the upper portion of the form for reporting instructions regarding your anticipated date of service. Please return the completed Part B and C questionnaire in the envelope provided.
Failure to appear for service when instructed to do so can result in a Show Cause Hearing appearance as well as a possible contempt of court charge.

Two-Step Questionnaire/Not a Summons
A two-step questionnaire cannot be completed online. A two-step questionnaire indicates “this is not a summons” in red. Two-step questionnaires must be returned via mail in the envelope provided. Those jurors that have received a two-step questionnaire may receive a summons at a later date.
Juror Reporting Instructions
Three locations are used for jurors reporting to Third Circuit Court. Your reporting location and Group number are found on the upper left portion on page one of your summons form. The Group number is used to identify the jurors requested to report. Please select the link below that corresponds to the location to which you are summoned:
Juror Reporting Times & Locations
Jurors should report to the appropriate location at the specified time. The links below provide directions to each location as well as DDOT and SMART Bus routes and maps.
Term of Service
Jury service is for one day or one trial. When you appear if you are not selected for a trial, generally your jury service is completed that day. If you are selected, you will serve as a juror for the duration of that trial. The average length of a trial is 3-4 days.
