Non-Agency Filings

Adult Adoption: For adoptions involving an adoptee that is eighteen (18) years of age or older.
Guardian Adoptions: For individuals who have guardianship of a child they wish to adopt. This includes relatives who are currently guardians.
Relative Adoption: For individuals wishing to adopt a relative child, and are not currently guardians.
Stepparent Adoptions: For stepparent to adopt with a child's custodial parent.

Attorney Instructions: Additional requirements when a party is represented by an attorney.
Adoption Report: Required to Establish a New Michigan Birth Record (DCH-085).

Section 45 (Competing Parties) Motion: For individuals who were denied consent to adopt from the Michigan Children’s Institute, and are seeking to challenge the denial and petition the Court to adopt the child(ren).
Motion for Delayed Registration of Foreign Birth: For individuals who adopted a child in a foreign country and are now seeking the creation of a Michigan birth certificate.
Petition Permanent Court Ward (Court to Consent): Must be completed with a Guardian or Relative adoption petition where a parent will not consent to the adoption. If you are filing a guardian adoption, you must have an order from probate court that granted the guardianship, giving you the Authority to Petition the Court to Adopt the child. If you do not have it, do not complete this form. You cannot file for a guardian adoption.
